Referee Consent form - to be signed by both teams prior to the kick-off and retained by the home team Secretary.
Referee's post-match form - to be sent to Gary Chapman, League Assistant Secretary
Suspensions - Clubs are now required to merely email Jamie Thomas to advise player suspensions (2020-21)
Benevolent Claim Form - Requests must be made to the Treasurer. We would also recommend that you apply to the GFA.
New Club (or Team) Application Form - to be sent to the League Secretary by 30 April
